Self-report after completing eligible activities on the member website or by calling us.
Step 3: Enjoy rewards
Redeem your rewards for a digital or physical gift card from CVS Pharmacy®, Home Depot, Albertsons, Subway and more. Receive digital gift cards within 48 hours.

| Get an eligible vaccine: annual flu shot, COVID-19 vaccine or booster, hep-B vaccine or pneumococcal vaccine | $20 |

Members can earn rewards by completing some or all program activities. Rewards can be earned from January 1 to December 31, 2024. Participation in the rewards program is voluntary and does not affect a member’s MA/MAPD plan benefits.
